Thank you for choosing Allura Health & Wellness (“Allura,” “we,” “us,” “our”).
By accessing our website, booking an appointment, receiving a service, or digitally signing this agreement, you (“Client,” “you,” “your”) agree to the following Terms of Service. This document governs all treatments, therapies, consultations, programs, memberships, and purchases at Allura, both in-clinic and online.

These Terms constitute a legally binding agreement. Please read carefully.

1. GENERAL TERMS & CONDITIONS

1.1 Scope of Services

Allura Health & Wellness provides integrative wellness services including:

  • Aesthetic & cosmetic treatments

  • IV Vitamin Therapy & hydration

  • Functional nutrition consultations

  • Holistic & integrative modalities

  • Bioresonance & META Hunter analysis

  • Hydrogen inhalation & hydrogen water therapies

  • Recovery therapies (compression boots, EMS, H-Wave, lymphatic modalities)

  • Weight-loss protocols (including tirzepatide)

  • Wellness memberships

  • Nutraceutical and wellness product sales

Some services require a licensed medical provider; others require a wellness practitioner. You acknowledge that staff roles may vary depending on the service provided.

1.2 Non-Emergency Disclaimer

Allura is not an emergency medical facility.
If you are experiencing chest pain, severe symptoms, or medical distress, call 911 immediately.

1.3 Eligibility

You must be:

  • At least 18 years old (or have a legal guardian present)

  • Able to provide truthful medical history

  • Not under the influence of alcohol or illegal substances

  • Not withholding medical conditions relevant to treatment

1.4 Clinic Policies

You agree to:

  • Arrive on time

  • Follow pre- and post-treatment instructions

  • Notify providers of changes to your health

  • Use your real identity when signing and booking

  • Treat staff respectfully

We reserve the right to refuse service for safety or behavioral concerns.

2. BOOKING, PAYMENT, CANCELLATION, AND REFUNDS

2.1 Booking

Appointments are booked through:

  • Our website

  • Phone

  • In-clinic

  • QR code booking pages

  • AI/SMS scheduling system

2.2 Payment Terms

You agree that:

  • All services must be paid in full at the time of booking or before treatment begins.

  • Specialty services (injectables, IVs, advanced diagnostics) may require a non-refundable deposit.

  • Packages, memberships, and promotions are not transferable.

2.3 Cancellation & No-Show

  • Cancellations within 24 hours may incur a fee.

  • No-shows may result in 50–100% service charge.

  • Late arrivals may shorten the appointment without refund.

2.4 Refund Policy

Due to the nature of health and aesthetic services:

  • All sales are final.

  • Services, packages, and memberships are non-refundable.

  • Remaining package sessions cannot be exchanged for cash.

  • Products (nutraceuticals, hydrogen devices, supplements) are non-refundable once opened.

  • Refunds are considered only for documented medical contraindications, with a provider’s letter.

3. MEDICAL HISTORY, CONSENT & ACKNOWLEDGEMENTS

3.1 Accurate Information

You affirm that all medical history and intake information you provide is true and complete.
Withholding medical information may increase risks.

3.2 Results Disclaimer

You understand that:

  • Individual results vary

  • No outcomes are guaranteed

  • Therapies are supportive and not substitutes for medical treatment

  • Integrative therapies complement but do not diagnose, treat, or cure disease

3.3 Provider Discretion

Your provider may:

  • Modify or refuse treatment if unsafe

  • Request medical clearance

  • Stop treatment mid-session if necessary

3.4 Photos & Records

Before-and-after photos may be required for certain treatments. You consent to clinical use for medical documentation. Marketing use requires separate written consent.

4. DEPARTMENT-SPECIFIC TERMS

Below are service-specific disclaimers and requirements.

A. AESTHETIC & COSMETIC SERVICES

Includes: Botox, fillers, microneedling, facial treatments, chemical peels, PRP/PRF, plasma fibroblast, skin analysis, and similar services.

A.1 Medical Nature

Injectables are medical aesthetic procedures performed by licensed providers (ARNP, PA, RN under supervision).

A.2 Risks

You understand possible side effects including:

  • Swelling, bruising, redness

  • Asymmetry, lumps, vascular complications

  • Infection, allergic reaction

  • Temporary discomfort

A.3 No Guarantees

Aesthetic results vary based on skin condition, lifestyle, metabolism, and compliance with aftercare.

A.4 Aftercare

Failure to follow aftercare may affect results and increase risk.

B. IV VITAMIN THERAPY & HYDRATION

B.1 Medical Screening

IV therapy requires:

  • Completed intake

  • Medical provider approval

  • Possible labs depending on service

B.2 Risks

You acknowledge risks including:

  • Bruising, infiltration, vein irritation

  • Lightheadedness, nausea

  • Allergic reaction

  • Infection

B.3 No Medical Claims

IV vitamins are wellness-supportive; they are not FDA-approved to diagnose or cure disease.

C. HOLISTIC THERAPIES

Includes META Hunter, bioresonance, microscopia/dark-field review, chakra/balance scans, and related modalities.

C.1 Not a Medical Diagnosis

These are wellness assessments and do not replace medical exams, labs, or diagnostics.

C.2 Purpose

Holistic sessions:

  • Provide energetic or terrain-based interpretations

  • Suggest lifestyle adjustments

  • Support functional wellness

No disease diagnosis or treatment is implied.

D. HYDROGEN THERAPY

Includes inhalation, hydrogen-rich water, PEM devices, and wellness protocols.

D.1 Wellness Modality

Hydrogen therapy is considered supportive and complementary.
Allura does not claim hydrogen cures or treats any condition.

D.2 Safety

Clients must disclose:

  • Respiratory issues

  • COPD

  • Severe anemia

  • Any acute symptoms

E. NUTRITION & FUNCTIONAL WELLNESS

Includes functional nutrition consults, lifestyle counseling, and HeadsUp Health tracking programs.

E.1 Not Medical Nutrition Therapy

These services:

  • Are educational

  • Do not diagnose disease

  • Do not replace primary medical care

E.2 Food, Supplement, and Habit Tracking

Participation may require daily check-ins, logs, or biomarker monitoring.

F. RECOVERY & PERFORMANCE THERAPIES

Includes compression boots, EMS/electrostim, mobility therapy, stretching, sports massage, H-Wave, lymphatic recovery.

F.1 Not Physical Therapy

These are recovery and performance services, not medical treatment.

F.2 Risks

Possible temporary soreness, fatigue, redness, or muscle sensitivity.

G. WEIGHT-LOSS PROGRAMS (INCLUDING TIRZEPATIDE PROTOCOL)

G.1 Independent Provider

Tirzepatide is administered by a nurse practitioner under her own medical licensure.
Allura provides the environment, screening protocol, and wellness support but does not hold or dispense controlled medications unless expressly stated.

G.2 Consent Requirements

All clients must sign:

  • Tirzepatide consent

  • Telehealth consent if applicable

  • IV nutrition protocol acknowledgment if part of the plan

G.3 Risks

You acknowledge risks including nausea, GI issues, low blood sugar, and other medication-related effects.

5. DIGITAL SIGNATURE & ELECTRONIC RECORDS

By signing digitally:

  • You agree your signature is legally binding

  • You authorize Allura to store your documents electronically

  • You consent to SMS/email communication related to your care

6. PRIVACY & DATA PROTECTION

6.1 HIPAA

Allura complies with applicable privacy standards.
Your private health information is not shared without consent except as required by law.

6.2 Data Collection

We may collect:

  • Intake forms

  • Health history

  • Biometrics

  • Progress photos

  • Appointment logs

  • Payment records

6.3 Marketing Consent

Marketing use of photos or testimonials requires a separate signed consent.

7. LIABILITY LIMITATION

To the fullest extent permitted by law:

  • Allura is not liable for unforeseeable or indirect damages

  • You accept the inherent risks of wellness treatments

  • You release Allura and its staff from claims related to:

    • Allergic reactions

    • Undisclosed medical history

    • Failure to follow aftercare

    • Expected or typical treatment responses

You agree to seek immediate medical help if experiencing concerning symptoms after any treatment.

8. GOVERNING LAW

These Terms are governed by the laws of the State of Florida.

Any disputes will be resolved through mediation or arbitration in Miami-Dade County.

9. ACKNOWLEDGMENT

By signing digitally, booking, or attending an appointment, you confirm:

✔ You agree voluntarily and without coercion
✔ You understand the nature and limitations of Allura’s services
✔ You accept responsibility for providing accurate health information
