Introduction

At Allura Health & Wellness, we take pride in showcasing real transformations, educational material, and authentic patient experiences. This Photo & Video Release Agreement explains how we may use images or videos captured during your visit.

By reading this page and acknowledging below, you confirm that you understand these terms and voluntarily consent to the potential use of your media as described.

1. Consent to Photography & Video

By proceeding, you acknowledge and agree that:

  • Allura Health & Wellness may photograph, film, or digitally record you during or after your treatment, for purposes such as before-and-after documentation, progress monitoring, or clinic operations.

  • These images may include photos, digital images, video, audio, and any comparable media (collectively, “Media”).

  • All Media may be captured by Allura staff, contractors, or authorized representatives.

You understand that you are never required to participate in photos or videos to receive treatment. Participation is entirely voluntary.

2. Intended Use of Media

You understand and agree that the Media may be used in the following ways:

Marketing & Promotion

  • Social media platforms (Instagram, Facebook, TikTok, YouTube, etc.)

  • Digital advertisements, promotional campaigns

  • Website content and landing pages

  • Email newsletters

  • Printed marketing materials (flyers, posters, brochures)

Educational & Informational

  • Internal staff training

  • Public educational content

  • Before-and-after galleries

  • Demonstrations of procedures or results

  • Blog posts or informational articles

Brand & Clinic Documentation

  • Treatment progress tracking

  • Transformational showcases

  • Internal visual records for service quality and development

You understand that your name will not be displayed unless you give additional written permission. Allura will take care to maintain professional and respectful representation in all uses.

3. Privacy Considerations

While Allura will take reasonable steps to avoid showing identifiable personal information, you acknowledge that:

  • Your face or recognizable features may appear in the Media.

  • Despite de-identification efforts, someone may still recognize you.

  • Published Media can be shared or reposted by third parties outside of Allura’s control.

4. Rights Granted to Allura

You grant Allura an irrevocable, royalty-free, perpetual, worldwide license to:

  • Use

  • Reproduce

  • Publish

  • Display

  • Modify

  • Distribute

the Media, in any format or platform now known or later developed.

You acknowledge that:

  • You will not receive compensation (financial or otherwise).

  • Allura owns all rights to the Media created.

  • All use of Media is at Allura’s discretion.

5. Revocation of Permission

You may revoke permission at any time by submitting a written request to:

However:

  • Revocation does not apply to Media already published, printed, scheduled, or in production at the time your request is received.

  • Previously used Media may continue to exist online or in physical formats indefinitely.

6. No Impact on Care or Services

Your decision to allow or decline Media use:

  • Will NOT affect your treatment, access to services, or relationship with Allura.

  • Will not influence pricing, appointments, or the care you receive.

Your participation is optional and welcomed only if you are comfortable.

7. Release of Liability

By consenting, you release Allura, its employees, directors, contractors, and representatives from any claims related to the use of the Media, including:

  • Invasion of privacy

  • Defamation

  • Rights of publicity

  • Misappropriation

  • Emotional distress

  • Any other claims arising from the recording or publication of the Media

You accept full responsibility for understanding the nature of digital media and publication.

8. Minors (Under Age 18)

If the client photographed is a minor:

  • Consent must be provided by a parent or legal guardian.

  • The guardian assumes responsibility for reviewing and approving this Release on behalf of the minor.

9. Governing Law

This Agreement is governed by the laws of the State of Florida.
Any disputes shall be resolved in Miami-Dade County.

Acknowledgment

By proceeding, you acknowledge that:

  • You have fully read and understand the terms of this Media Release Agreement.

  • You are voluntarily giving your consent.

  • You agree to the use of Media as described in this document.
