Membership & Eligibility

• A qualified healthcare provider will determine whether you are an appropriate candidate for any treatment.

Important Safety Information – BOTOX® Cosmetic

BOTOX® Cosmetic is a prescription treatment administered by qualified medical professionals. As with any medical procedure, it may cause side effects.

Seek medical attention immediately if you experience:

• Difficulty swallowing, speaking, or breathing

• Muscle weakness beyond the treatment area

• Vision changes, drooping eyelids, or double vision

• Hoarseness or difficulty speaking

• Allergic reactions such as rash, wheezing, or dizziness

Do not receive BOTOX® Cosmetic if you:

• Are allergic to botulinum toxin products

• Have an infection at the injection site

Inform your provider if you have:

• Neuromuscular conditions (e.g., myasthenia gravis, ALS)

• Recent facial surgery or planned procedures

• Pregnancy or breastfeeding

• Received botulinum toxin injections in the past 4 months

• Taken muscle relaxants, antibiotics, blood thinners, or sleep medications

Common side effects may include:

• Injection site discomfort

• Headache or neck pain

• Temporary eyelid drooping

• Dry eyes or blurred vision

Approved Uses

BOTOX® Cosmetic is used to temporarily improve the appearance of:

• Forehead lines

• Crow’s feet

• Frown lines between the eyebrows

• Vertical neck bands (platysma bands)
